Metformin side effects, from the label

The metformin tablet label, revised July 2025, reports what people had in a placebo-controlled trial and what to do about it. Here it is in plain words.

Stop taking metformin and call your prescriber right away if you feel very weak and tired, have unusual muscle pain, trouble breathing, unusual sleepiness, unexplained stomach problems with nausea, vomiting, or diarrhea, feel cold especially in your arms and legs, feel dizzy or lightheaded, or have a slow or irregular heartbeat. Those are the label's signs of lactic acidosis, the reaction its boxed warning is about. The label says the onset is often subtle, with nothing more specific at first than feeling unwell, achy, or short of breath.

The common ones, and how common

Out of every 100 people on metformin in the label's trial, compared with 100 on placebo:

  • Diarrhea: 53, against 12 on placebo
  • Nausea or vomiting: 26, against 8
  • Gas: 12, against 6
  • Weakness: 9, against 6
  • Indigestion: 7, against 4
  • Stomach discomfort: 6, against 5
  • Headache: 6, against 5

Diarrhea was bad enough that 6 in 100 stopped taking it.

How long it lasts

The Medication Guide says these effects "generally go away after you take the medicine for a while." It does not name a number of weeks, and nothing here should.

What the label does describe is the dose being raised in steps, 500 mg a week or 850 mg every two weeks, based on how your blood sugar responds and how well you tolerate it. If a step up brings the stomach trouble back, that is a thing your prescriber wants to hear about, because the pace is theirs to set.

What the label says helps

  • Take it with meals. The label says this to lessen the upset stomach. MedlinePlus adds that the immediate-release tablet is usually taken with meals two or three times a day; yours reads however your prescriber wrote it.
  • A missed dose: the Medication Guide says take your next dose as prescribed and do not take an extra dose the next day. MedlinePlus gives a different rule (take it when you remember unless the next dose is close), so ask your prescriber which they want you on and follow that one.

The vitamin B12 part

In the label's 29-week trials, about 7 in 100 people had their vitamin B12 drop below normal. It reversed when metformin was stopped or B12 was supplemented. The label calls for a B12 check every two to three years and a blood count every year. If nobody has mentioned it, it is a fair question at the next visit.

Who the label says needs extra care

The label's boxed warning is about lactic acidosis, and it lists who is more at risk: reduced kidney function, age 65 and over, a scan with contrast dye, surgery, a liver problem, and heavy drinking. Two practical things follow. Tell any office scheduling a scan or a procedure that you take metformin, because they may pause it. And ask your prescriber where they want you on alcohol.

What I do with all this, and what I don't

I never change your dose, never tell you to skip one, and never diagnose. What I do is remind you at the meal, ask every day how your stomach is, keep the answers, and when your prescription printout is on file hold what you tell me against its stop-and-call list so a bad turn gets noticed on the day it happens.
