Zepbound side effects, from the label

Zepbound (tirzepatide) is a once-weekly injection. Its prescribing information, revised January 2026, lists what people reported in the trials and how often. Here is that list in plain words.

Stop the medication and get medical help right away for swelling of the face, lips, tongue, or throat, trouble breathing or swallowing, a severe rash, fainting, or a very rapid heartbeat. Severe stomach pain that will not go away, with or without vomiting, sometimes spreading to the back, is also a stop-and-call-right-away symptom: it can be pancreatitis.

The common ones, and how common

Out of every 100 people on Zepbound in the two main trials, compared with 100 on placebo:

Nausea: 25 to 29, against 8 on placebo
Diarrhea: 19 to 23, against 8
Constipation: 11 to 17, against 5
Vomiting: 8 to 13, against 2
Stomach pain or indigestion: about 9 to 10 each, against 4 to 5
Injection site reactions: 6 to 8, against 2
Tiredness: 5 to 7, against 3
Hair loss: 4 to 5, against 1
Heartburn, burping, dizziness: about 4 to 5 each, against 1 to 2
Bloating and gas: 3 to 4 each, against 2

Counting every stomach-related effect together, 56 in 100 had at least one, against 30 in 100 on placebo. The ranges are the 5 mg, 10 mg, and 15 mg doses.

When they hit, and when they ease

The label says most of the nausea, vomiting, and diarrhea happened during dose escalation, the weeks after a step up, and decreased over time. About 2 in 100 on the lowest dose and about 4 in 100 on the highest stopped Zepbound because of stomach effects, most of them in the first few months.

So a rough week on a new dose is what the label describes. Its own line for when to call is stomach problems that are severe or will not go away, and not being able to keep fluids down belongs in that call too.

What the label actually says to do

Drink fluids. Diarrhea, nausea, and vomiting can dehydrate you, and the label ties dehydration to kidney problems. This is the one piece of self-care it spells out.
With or without food. Zepbound can be taken either way. The label does note it can make your stomach empty more slowly.
A missed dose: take it as soon as possible within 4 days. Past 4 days, skip it and take the next one on schedule. Never take two doses within 3 days of each other.

The smaller-meals and low-fat advice you will see elsewhere is not in the label. Ask your prescriber or dietitian what they want you to do on the queasy days, and follow that.

Call your prescriber about

A lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath. The label carries a boxed warning about thyroid C-cell tumors seen in rats; whether that applies to people is unknown, and Zepbound is not prescribed to anyone with a personal or family history of medullary thyroid cancer or MEN 2.
Stomach problems that are severe or will not go away.
Right away: pain in the upper stomach, fever, yellowing of the skin or eyes, or clay-colored stools, which can mean a gallbladder problem.
Right away: any change to your mood or mental health that is new, worse, or worries you.
Call 911 for a seizure, loss of consciousness, or trouble breathing.

What I do with all this, and what I don't

I never change your dose, never tell you to skip one, and never diagnose. What I do is ask every day how the nausea is, keep your answers, and when your pharmacy printout is on file hold what you tell me against its call-the-prescriber list so a bad turn gets noticed on the day it happens.
